For a license under a fictitious business name, the application shall include a certified copy of the fictitious business name statement filed with the County Clerk. Send the application package to the Bureau of Security and Investigative Services, P.O. Box 989002, West Sacramento, CA 95798-9002. You will receive a copy of the shurlok real estate lock boxes Act with your license.
